Personal cards of workers (form T-2) Used in organizations for full and comprehensive accounting of personnel. At their foundation, a personnel card is formed. Unified form No. T-2 is used as a form, which reflects all the necessary information about the personality of the employee and its work.

The form consists of 4 pages containing thematic blocks. The first and second pages contain general information about the employee and are filled with its employment; The third and fourth - fill in the process of work.

In the T-2 card, there must be a signature of the employee:

1. What document approved a personal card?

2. Does all employers make T-2 cards?

5. Based on what documents should T-2 be filled when driving?

  • employment contract concluded with an employee;

  • passports or other identity document;
  • labor book or document confirming work experience;

  • document on education, on qualifications or availability of special knowledge - when taking a job requiring special knowledge or special training;
  • information reported by the employee.

In some cases, taking into account the specifics of the work in accordance with the current legislation of the Russian Federation, the employer has the right to require additional documents.

It is forbidden to require documents from the person who goes to work, in addition to the Labor Code of the Russian Federation,), other federal laws, decrees of the President of the Russian Federation and the Resolution of the Government of the Russian Federation (Article 65). 7. How many years should the personal cards be stored?

You should not use when filling the card, such characters as colon (:), equality (\u003d), oblique line (/). In the coding zone, such designations are generally unacceptable.

If the personal card is not filled with some data or answers are negative, we do not write anything in the encoded part. Diggers in empty graphs are not put.

The primary filling of the personal card can be carried out or manually or in electronic form. Subsequent records are already manually entered, since the employee must be familiar with them on receipt, and the reprint of the T-2 form is prohibited in a new way.

Fill out the "cap" of the P-2 personal card

In the header of the personal card, you specify the OKPO organization on the basis of statistical data. The name of the organization is written without abbreviations. The location of the company is written through the comma after the name.

Personal accounting card starts at work immediately after registering an order for employment. At the same time, you need to record an employee's employment record.

Personnel Number - This is an employee number who is assigned to him at this enterprise for internal accounting. When filling out a personal card, the table number has already been assigned to him (from the order of the head).

Inn (taxpayer identification number). The employee may not provide the INN when the device is to work, because This is not spelled out in the TC, nor in the NK, and, accordingly, is not mandatory. If the Inn does not have an employee or he did not provide it, this field is not filled.

SNILS (insurance certificate of state pension insurance).

Alphabet. Here you specify the first letter of the employee's family for the convenience of systematization of cards.

Character of work. We specify "constantly" or "temporarily". Fill on the order basis. Words write completely.

Type of work. We indicate the main work or part-time. We write completely.
If the employee works on internal part-time, the personal card is not filled with it, since all information is specified in the card at the main place of work.

Section I "General Information"

1. Surname, name, patronymic. These data are recorded on the basis of a passport. We write completely and picking up.

The encoding zones are mandatory on the basis of all-Russian classifiers:

2-3. Date and place of birth Writing on the basis of a passport or similar document. We write in complete words, for example: "May 24, 1973" and we encode 24.05.73.

Row "Place of birth" when filling should not exceed 100 characters. Areas and regions we write in the parental case, without commas. 4. Citizenship we write without a reduction. Citizenship is subject to encoding on the basis of OKIN:

Citizen of the Russian Federation - 1;

- a foreign citizen (indicated by which state) - 3;
- a person without citizenship - 4.

If an employee has two citizenship and the second, he did not inform the employer, it is not the fault of the employer. This information is required only for workers who are arranged to work related to advent of state secrets.

5. Language knowledge

In this point, write a foreign language in relation to the language of Russia, which is owned by the worker, if necessary for work. The data take from the words of the employee itself (questionnaire, test, etc.).

The personnel inspector can fill out two codes: the language itself and the degree of its knowledge, and may indicate only one code - the degree of knowledge of the language. These data are recorded in several gaps. Language codes and degrees of their knowledge:
- English (OKIN code "014"), German (OKIN code "135"),
- French (OKIN code "213") Italian (OKIN code "070");
- reads and translates with the dictionary - code by OKIN "1";

If the employee owns several languages, they are recorded separately in each row.

6. All information about education. Here we write qualifications, specialty, etc. This data take from the formation documents.

Education has the following OKIN encodings:
- initial (general) education - 02;
- Basic general education - 03;
- secondary (full) general education - 07;
- primary vocational education - 10;
- secondary vocational education - 11;
- incomplete higher education - 15;
- Higher education - 18;
- Postgraduate education - 19.

The T-2 form has two blocks of graph, which indicate information about two educational institutions.

If the employee graduated from three courses of the university, they belong to persons with incomplete (unfinished) higher professional education. Confirmation of such education is only a certificate or a diploma of incomplete higher education of the state sample (not a diverting and a student card!).

If the employee did not finish the university, then his education records will look as follows.

If the employee was trained in fully or more than half, but did not defend the thesis or did not pass the state. Example - "incomplete higher";

If an employee graduated from three courses - "incomplete higher, III (IV, V, VI) course";

If an employee studied, but did not finish university or vocational school and passed less than half of the courses, then education is writing on the previous ended educational institution: "basic general"; "Average (full) general"; "lower secondary"; "Unfinished secondary education".

If the employee receives the formation of the following level, then education is added additionally in part 1 of clause 6, for example:

The name of the educational institution, its series and number we write on the basis of a document on education. Long names permissible to cut without loss of meaning (for example, name - "name"). Qualification (degree) is written on the basis of the same education document. To qualify "Bachelor" and "Master", the direction is indicated, and for the qualification "Specialist" - specialty.

The profession code is defined by OKPDTR and includes 17 digits, and position code - 13. The code contains all the information about the profession of the employee, working conditions, positions and payment. It is allowed to reduce the code to five or six first digits that denote the profession (5 digits) or profession and control number (6 digits).

8. Work experience. The experience take from records in the workbook (employment contracts, etc.). It is reflected in the days, months, years.

The duration of employment is determined as the difference in the sums of the end of the end and the beginning of the periods specified in the employment recorders. Since the day of dismissal is the last day of work, it should be added to the total amount of the experience. A calculated overall work experience is written in total - this is the number of years, months and days that worked worker.

Section IV "Certification"

In this section, fill data from the decision of the attestation commission, which are usually formulated as follows:
- complies with the post;

- does not correspond to the position.

We write the date of certification, the number and date of the results of the certification, the basis for the certification (order).

Section V "Raise Qualifications"

Information on advanced training and professional-statement is filled on the basis of documents provided by the employee, or from the personnel training department. Fill out:

Section VIII "Vacation"

This section contains information about all vacations that have been provided to the employee: regular leave, child care leave, vacation without detention.

Section IX "Social Benefits"

Section X "Additional Information"

This section is filled at the discretion of the head. You can include information that has not been reflected in other departments. Change of employee information

If an employee has changed (changes in the status of personal life, addresses, passport data, etc.), they are indicated by his personal card and are assigned by the inspector signature and employee itself. At the same time, the old record is crossed out and the same graph is made new on the basis of new documents provided.

It is undesirable to change a personal card due to the fact that it is difficult to collect all the signatures from the employee and restore its initial data.

For example, if the employee has changed the surname, then you can write a new one: cross it old, and write a new one from above or on the side. This entry is assigned by the employee's signature of personnel here.

Note. Personal card of the state (municipal) employee (the form of T-2 GS (MS)) is applied to accounting for persons replacing state (municipal) posts of civil service.

Since personal cards contain personal data of employees, they must be stored in a safe or wardrobe locked.

Note that some employers based on the unified form T-2 develop their form in electronic form, while removing individual graphs that are subsequently printed. So it is impossible to do! The procedure for applying unified forms of primary accounting documentation approved by the Resolution of the State Statistics Committee of the Russian Federation dated 24.03.1999 N 20, it was found that the removal of details from unified forms is prohibited. If necessary, changes can be made to a unified form:

- expansion and narrowing of the graph and rows, taking into account the number of characters in the indicators;

- inclusion of additional lines and deposit sheets;

Filling rules and procedure for making information

At the top of the first page form, you must specify the name of the organization that is taken from constituent documents. We now turn to the table.

In the "Date of Compilation" column, you specify the date of filling the personal card. Note that it may differ from the recruitment date of the employee to work. This is not a violation of labor legislation. Next in the form of T-2 there are a number of fields for encoding. Why are they? These codes were required for statistical observation and control. Currently, these fields are extremely rarely filled, but if yet in your institution it is customary to encode information, we recommend using the following classifiers:

Section I "General Information"

When viewing this section, it can be seen that personal data of the employee and information about his family, the education obtained before the employment and the existing experience are made to it.

The date and number of the employment contract concluded with the employee are affixed in the "Labor Treaty" column.

Paragraphs 1-4 of this section are filled on the basis of passport data:

- Surname, name and patronymic are indicated in the nominative case completely;

Based on the staff submitted by the employee, paragraph 6 "Education" is filled:

- secondary vocational education - technical school or college;

Typically, information about completed education is indicated, but if an employee has a certificate of incomplete higher education, it can also be reflected in the personal card. At paragraph 7, "Profession" should make information about the main and additional professions. Details to fill this section of the personal card are taken from:

This section is filled with a new profession by mastering the employee. After completing the training program, over 500 hours is issued a diploma diploma, on the basis of which the entry is made.
